“My Labs Were Normal… So Why Do I Still Feel This Way?”

We hear this every day.

 

In conventional medicine, thyroid evaluation often relies on a single screening test: TSH. When hypothyroidism is advanced, TSH is clearly abnormal and the diagnosis is straightforward.

 

But for many women, this approach falls short.

Why Normal Labs Don’t Always Mean Optimal Thyroid Health

There are two major reasons symptoms persist despite “normal” labs:

 

1. Normal Is Not the Same as Optimal

Laboratory reference ranges are broad and based on population averages—not on what allows your body to function optimally.

 

2. You May Be Producing Hormone—but Not Using It

Even more commonly, the thyroid gland produces enough hormone, but the body cannot use it effectively at the cellular level.

When this happens, cells behave as if you are hypothyroid—creating symptoms—while TSH remains normal.

The thyroid primarily produces T4 (thyroxine), which circulates in the bloodstream but is relatively inactive.

For thyroid hormone to work, T4 must be converted into T3, the active form that tells cells to:

 

  • Burn fat

  • Produce energy

  • Regulate metabolism

 

This conversion occurs mainly in the liver and tissues.

If conversion is impaired, you may have adequate hormone levels—but insufficient cellular activation.
